Abstract: This article sorts out the theory origin of the "holistic" care model and hospice care social work, analyzes the applicability of the "holistic" care model to the hospice care of the elderly in residential care institutions, and theoretically summarizes the framework of hospice care social work. Then combined with the practice of the "road to peace" hospice care project in Chongqing Y Nursing Home, a "holistic" care practice model for hospice care social work is refined. That is to say, in the service of social work in hospice care, clients include the hospice elderly, elderly family members, volunteers and other team members. The content of the service is to provide "a better life for the deceased" from the preparatory period of the hospice to the late stage, the service of "goodbye of the relatives" from the late stage to the elderly dying period, and the service of the "good life of the left" after death.
